This position provides a coordinated approach for outreach to homeless and displaced individuals with behavioral health issues by searching and identifying homeless individuals on the streets, as well as in shelters and homeless programs. Will develop and deliver service-planning and personalized case management in order to connect individuals to resources available to meet their needs. Also provides for collaboration and communication among community partners. Experience & Knowledge: Bachelor degree required; previous experience working with persons with severe mental illness, drug and/or alcohol addicted and other service-resistant populations preferred; experience in working with the homeless population; direct experience with street outreach desirable; and experience in preparing and implementing Risk Management plans with clients. This multi-role position includes professional processes such as case management, data coordination, training/facilitating and grants management. Professional case management requires a high level of responsibility for providing high-level customer service throughout the implementation of the volunteer and child enrollment and matching process, match support and supervision to ensure child safety, positive outcomes for youth, and creating and supporting the development of constructive, satisfying relationships between children and volunteers.
