Health Care Navigator – Monroe, LA
Full-time position providing services that include connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care. Health care navigators provide case management and care coordination, health education, interdisciplinary collaboration, coordination and consultation duties. Health Care Navigators work closely with the Veteran’s primary care provider and members of the Veteran’s assigned interdisciplinary treatment team. Experience & Knowledge: LCSW, LMSW or LPC required; knowledge of community resources; professional experience in structured group facilitation skills; and at least two years of experience in a health care or social services practice preferred. Shelter Advocate – Monroe, LA                                                                                                                                                                                                 The shelter advocate is responsible for providing safe shelter for battered women and their children. The shelter advocate works to empower battered and abused women and their children to make choices that result in safe, independent and violence-free lives. Full time multi-role position includes professional processes such as program oversight, case management, data coordination, training/facilitating, grants management, and maintaining a small caseload, as needed. Responsibilities require a high level of responsibility for providing high level customer service, positive outcomes for youth, and creating and supporting the development of constructive, satisfying relationships between children and volunteers and community partners. Experience and Knowledge: Bachelor’s degree with major emphasis in social services, social work, psychology, sociology and case management experience. Outreach Specialist – Monroe, LA                                                                                                                                                                          Full time position providing a coordinated approach for outreach to homeless and displaced individuals with behavioral health issues by searching and identifying homeless individuals on the streets, as well as in shelters and homeless programs.  Will develop and deliver service-planning and personalized case management in order to connect individuals to resources available to meet their needs. Also provides for collaboration and communication among community partners. Experience & Knowledge: Bachelor degree required, Master degree preferred in social services or related field; previous experience working with persons with severe mental illness, drug and/or alcohol addicted and other service-resistant populations; experience in working with the homeless population, direct experience with street outreach desirable; experience in preparing and implementing Risk Management plans with clients; experience in engaging difficult populations.
